What it measures
Serum 25-hydroxyvitamin D level — the standard marker for vitamin D status. Distinguishes deficiency (<20 ng/mL), insufficiency (20–29), and sufficiency (≥30).
When it’s ordered
Suspected deficiency (fatigue, bone pain, muscle weakness), osteoporosis workup, malabsorption disorders, chronic kidney disease, post-bariatric surgery.
Medicare coverage
Governed by NCD 190.22 with a restrictive accepted-codes list. Routine screening in healthy adults is generally NOT covered. Common accepted codes: E55.9 (vitamin D deficiency), M83.x (osteomalacia), N18.x (chronic kidney disease).
Typical price
Medicare allowable around $40–45. Commercial cash prices range from $50 to $300.
