Is MedCarePrecheck free?
Yes. Pre-Check, Code Lookup, Rate Lookup, and Find a Lab are all free. You don't need an account.
How accurate is the coverage check?
We use the official CMS NCD/LCD data and CPT/ICD-10 code tables. The check is as accurate as Medicare's published rules — but final coverage depends on your specific plan, prior test history, and what your doctor documents. Always confirm with your provider.
What is an ABN?
An Advance Beneficiary Notice (ABN) is a form the lab asks you to sign that says: 'Medicare may not cover this — if they don't, you owe us.' You can decline to sign, but the lab may refuse to perform the test. See the ABN Rights page for your options.
My doctor used Z00.00 — is that a problem?
Z00.00 is the code for a 'general adult medical examination'. Medicare often won't pay for individual lab tests under this code because it doesn't indicate medical necessity. A more specific code (like one for high cholesterol, diabetes, or anemia) usually works.
What's the difference between an NCD and an LCD?
An NCD (National Coverage Determination) is a Medicare rule that applies nationwide. An LCD (Local Coverage Determination) is set by your regional Medicare contractor and only applies in certain states. We check both.
Why are some tests shown as 'no specific rules on file'?
Common panels like the Comprehensive Metabolic Panel or Lipid Panel aren't restricted by Medicare to a specific diagnosis-code list. Coverage is judged case-by-case under general medical-necessity rules. Any code that reflects your actual condition should be acceptable.
Do you store my lab order?
Yes, briefly — we keep a redacted copy in our QC archive for 30 days so we can investigate any errors users flag. After 30 days it's automatically deleted. We never sell or share your data.
I already got a bill. Can MedCarePrecheck help me appeal?
